Possibility on Insulin-with Non-Insulin-Dependent Diabetes Mellitus



Abstract
Background:
@EN The indications for starting treatment with insulin have been often described in type II diabetics, but there has been little informations about whether to treat with insulin in type II diabetics with insulin therapy. To identify patients
with
NIDDM
for whom insulin therapy is most beneficial and to determine if the requirement for insulin could be perdicted, we performed a study among patients with NIDDM.
@ES Methods:
@EN Asymptomatic, insulin-treated patients were recruited and performed diet education and baseline studies. Over 12 weeks, patients developing hyperglycemic symptoms or ketonemia were counted as insulin withdrawal failures. We withholded insulin
injections in 20 patients at initial visit and examined the patients every 4 weeks.
@ES Results:
@EN Among the 20 patients entered the study, 15 patients were evaluated through 12 weeks and 6 patients could withhold the insulin treatment and failure criteria developed in 9 of 15 insulin-withdrawal patients. There was no difference in
clinical
and
laboratory characteristics such as age, insulin dosage, duration of insulin use, blood glucose level and fasting or pp2hr C-peptide levels except fructosamine between success and failure groups.
@ES Conclusion:
@EN Among the patients with NIDDM who were on insulin treatment, some of patients could be managed without insulin. However there was not possible factors to predict successful withdrawal of insulin.
